RELEASE OF LIABILITY, WAIVER OF CLAIMS & ASSUMPTION OF RISK

Please read carefully before signing. By signing this agreement, you waive certain legal rights, including the right to sue or claim compensation.

This agreement applies to all participation in activities, services, events, and use of the facilities, smash rooms, tools, equipment, and premises operated by Unleashed Rage Rooms Inc. (“Activities”).

By participating, I acknowledge and agree to the following:

ASSUMPTION OF RISK

I understand that participation in rage room activities involves inherent risks, including but not limited to:
- Slips, trips, falls, and collisions
- Flying debris, broken glass, and sharp objects
- Equipment malfunction or misuse
- Physical exertion and bodily injury
- Negligence of other participants
- Failure to follow staff instructions
- Damage to personal property
- Serious injury, disability, or death

I voluntarily accept all risks associated with participation in the Activities.

MEDICAL & SAFETY CONFIRMATION

I confirm that:
- I am physically and mentally capable of participating safely
- I am not under the influence of drugs or alcohol
- I have disclosed any medical conditions that may affect my participation
- I understand participation may not be suitable for individuals with certain medical, physical, or mental health conditions, including pregnancy

I agree to follow all staff instructions, safety procedures, and equipment requirements at all times.

RELEASE OF LIABILITY

In consideration for being permitted to participate, I agree to:
- Waive any and all claims against Unleashed Rage Rooms Inc., its owners, employees, contractors, volunteers, affiliates, and representatives (“Releasees”)
- Release the Releasees from liability for injury, death, property damage, or loss arising from participation, including claims resulting from negligence
- Indemnify and hold harmless the Releasees from any claims, legal actions, damages, or expenses related to my participation

MEDIA & PUBLICITY RELEASE

I understand that photos or videos may occasionally be taken during activities for promotional or social media purposes. By participating, I grant permission for my likeness to be used by Unleashed Rage Rooms Inc. unless I request otherwise in writing prior to participation

GENERAL TERMS

I acknowledge that:
- Participation is voluntary
-Age Requirement: Participants under 18 years of age must have this waiver signed by a parent or legal guardian. Minimum participant age is 9 years old.
- This agreement is governed by the laws of the Province of Manitoba, Canada
- Any legal proceedings must be brought exclusively within Manitoba

I confirm that I have read, understood, and voluntarily agree to this Release Agreement and understand that I am giving up certain legal rights by signing.
