Release of Liability, Waiver of Claims, Assumption of Risks, Indemnity, and Publicity Release Agreement

(Hereinafter, the “Release Agreement”)

Please read this Release Agreement carefully. By signing, you waive certain legal rights, including the right to sue or claim compensation following an accident. The undersigned releases Unleashed Rage Rooms Inc., its directors, officers, employees, guides, agents, representatives, volunteers, independent contractors, subcontractors, sponsors, successors, and assigns (collectively, the “Releasees”) as set forth in this agreement. The term “Activities” refers to any use of the Smash Room facility, related equipment, or participation in any events, activities, or services provided, organized, or authorized by the Releasees.

Assumption of Risks

I understand that the Activities involve unusual risks, including but not limited to:

• Accidents in the facility, slips, and falls

• Malfunction of equipment, safety gear, or smashing tools

• Injury, open wounds, or aggravation of existing medical conditions

• Negligence by other participants or failure to follow employee instructions

• Shock, stress, or other bodily injury, including risks caused by negligence of the Releasees

I have received full information about the Activities, had the opportunity to ask questions, and freely accept all associated risks, including injury, property damage, or death.

Medical Conditions

The Activities place unusual stress on the body and are not recommended for individuals with:

• Asthma, epilepsy, cardio/respiratory disorders, hypertension

• Skeletal, joint, or ligament problems

• Certain mental health conditions

• Pregnancy or suspected pregnancy

I understand that I should consult my medical practitioner if I have concerns about participating.

Release of Liability and Indemnity

In consideration of being allowed to participate, I agree to:

1. Waive all claims and release the Releasees from liability for any injury, death, or property damage, including negligence.

2. Indemnify the Releasees for any claims or costs arising from my participation.

3. Acknowledge that participation is voluntary and I am not relying on any representations about safety beyond this agreement.

4. Confirm I am at least 18 years old and legally competent. If under 18, a parent or guardian must sign.

5. Confirm I have no medical or physical conditions preventing safe participation.

6. Confirm I have not consumed alcohol and am not under the influence of drugs or liquor.

7. Agree that this Release Agreement is binding on my heirs, executors, administrators, and assigns.

8. Agree that this Release Agreement is governed by the laws of the Province of Manitoba, Canada.

9. Consent that any legal action will be brought solely within the Province of Manitoba.

I confirm that I have read, understood, and agreed to this Release Agreement. I understand that by signing, I am giving up certain legal rights.
